This form is for all vendors interested in a spot with a table (6'), a tablecloth and 2 chairs under a shared vendor tent (10'x10') at the Saturday, August 22, 2026 Parkdale Music Crawl & Street Festival 2026. All food vendors MUST have an unexpired Ontario Food Handler Certificate and a Food Safety Inspection Reports (FSIRs from Public Health). Please upload your certificates here or contact Toronto Public Health for more information.

Please describe what you will be selling at your table. If you are not a Parkdale Village BIA member, we will be prioritizing handmade arts and crafts and food products (no onsite cooking, no alcohol, preferably room temperature) that Parkdale Village businesses do not offer. *

If selected, we will charge $100+HST ($113) for 1 table and 2 chairs for all of Saturday, Aug 22nd between 11:00 AM and 9:00 PM. Up to 2 vendors, each with their own table and chairs, will share 1 tent. Do you agree to be a WHOLE DAY and OUTDOOR vendor under a SHARED tent? *

Do you want 1 extra table and 2 extra chairs under the same tent for a total cost of $200+HST ($226)?
